  • Copy of SS4 Form (To verify company information)
  • Copy of Articles of Organization or Articles of Corporation (verify company information)
  • Copy of Owner's K-1s (ownership verification)
  • The last quarterly wage and tax report filed (UI3/40 form) 
    • Please reconcile this report to who is full-time, part-time, terminated, or seasonal
